How do you investigate a reputation error involving images, audio or video in an AI answer?

In brief

Preserve the original media and the version supplied to the product, then isolate the mistaken observation or inference. Identify whether the model was reading a screenshot, interpreting a chart, transcribing a name or summarizing an edited clip. Test the factual task involved in that specific step.

Compare the media with its caption, date, speaker identity, surrounding sequence and available transcript. Check whether a cropped graph lost its denominator, an old logo implied current ownership, or a transcript misidentified an executive. Gemini's image documentation describes interpretation of supplied images. For other AI products, investigate the assets and information path used in the observed answer.

Repair the relevant source layer: correct the caption or transcript, restore missing context, update an obsolete file, or report a fabricated or misattributed artifact. Keep a public factual reference where it helps verify the point. Test the affected input and realistic fresh research paths separately, retaining the exact files and outputs. This reveals whether the error arose in recognizing the media, connecting its identity or drawing a conclusion from it.
